Immunizations save lives and prevent the spread of many serious diseases. Providing and promoting immunizations is one of public health's greatest success stories.

Immunizations for Children & Adolescents

We participate in the Washington State Childhood Vaccine Program (CVP), which offers low-cost vaccinations to all children in Washington state regardless of insurance status. A Child Vaccine Program visit costs $20 per child regardless of how many vaccines are needed.

Immunizations for Adults

We participate in the Washington State Adult Vaccine Program, which offers low cost vaccination to adults 19 years and up who are either uninsured (currently have no health insurance) or underinsured (your current health insurance does not cover any of the cost of the vaccination you are seeking). An Adult Vaccine Program visit costs $20 per adult regardless of how many vaccines are needed.

TB Skin Testing

We offer low-cost skin testing for Tuberculosis (TB). A visit for a 1-step TB skin test, including the reading of the test is $35. If a 2-step test is required, the cost is $50. If follow up testing is required, we will refer out to other providers in the area to provide those services.

Clinic Fees

We do not bill to insurance. We accept cash, credit, or check for payment of services.

We offers sliding scale adjustment of fees based on monthly income using Federal Poverty Guidelines. To be eligible, you will need to show one of the following:

  • Proof of income in the form of a paystub or most recent W2
  • Proof of enrollment in a State Assistance Program (WIC Card, TANIF Letter, EBT Card, etc.)

  • Administration: $20/patient
    • Children & adolescent: $0/dose
    • Adult: $0/dose

TB Testing Fees

  • 1-step TB test & read: $35
  • 2-step TB test & read: $50

Immunization Records Release Request

Accessing immunization records for yourself or your child is an important aspect of healthcare management. Below are some available options for accessing immunization records:

Sign up for MyIR Mobile by going to myirmobile.com and follow the registration instructions. Your registration information will be used to match your records with the state immunization registry. You will be sent a verification code on your phone to finalize the process. Once registration is complete, you'll be able to view your immunization records and Certificate of Immunization Status (CIS).

Visit Your Local Pharmacy, Clinic, or School

Most healthcare providers, schools & pharmacies in Washington use the Washington State Immunization Information System. Please check with them to ask if they can give you a complete immunization record for you or your child. They can print it from the Immunization Information System or from their own medical record system.

To submit your request, submit an online Immunization Records Release Request. We may be able to complete the request online and send the records to an email, fax, or mailing address. Once a request has been submitted, we will reach out to you with more information.
